Kurzfassung
Ziel dieser Arbeit war die Evaluation eines Behandlungspfades zur Diagnostik von Synkopen. Dieser Algorithmus soll eine strukturierte Abklärung ermöglichen. Er basiert auf den aktuellen Richtlinien der Gesellschaft für Kardiologie (European Society of Cardiology) für das Diagnostik- und Therapiemanagment von Synkopen. Die bisherige Struktur des Universitätsklinikums Aachen war grundlegend und die notwendigen Untersuchungen wurden stratifiziert eingesetzt. Gegenstand der Prozessanalyse war die Effektivität hinsichtlich der Abklärung von Synkopen in der diagnostischen Qualität mithilfe der Bewertung durch ein mittleres Follow-Up zu prüfen. Des Weiteren sollte die Nutzung von Ressourcen und Liegezeit bewertet werden, um eine Aussage über die Kosten zu machen. Im nächsten Schritt wurde eine virtuelle Synkopenambulanz entwickelt und mit den vorliegenden Patientendaten evaluiert, ob durch eine schnelle Versorgung und Kurzzeitüberwachung von 24 Stunden eine Kostenoptimierung erreicht werden kann. In einem Zeitraum von sechs Monaten wurden 131 Patientenfälle über die Notaufnahme des Universitätsklinikum Aachen aufgenommen. In 100 Fällen wurde vollständig nach dem Algorithmus diagnostiziert. 68% der Patienten wurden dafür stationär und 32% ambulant aufgenommen. 96% der Synkopenursachen wurden abgeklärt, 4% blieben nach Ausschöpfung des Algorithmus ungeklärt. Im Follow-Up hatten 8% der Patienten ein Rezidiv. Keiner der ambulant behandelten Patienten erlitt ein Rezidiv und keiner der Patienten verstarb. Die Kostenanalyse ergab Gesamtkosten von 467 € für eine ambulante und 2389 € für eine stationäre Synkopendiagnostik. Durch die virtuelle Umorganisation der Synkopenambulanz hätten mehr Patienten ambulant untersucht werden können, ohne dass dies ein Sicherheitsrisiko bedeutet hätte. Die Kostenanalyse unter diesem Aspekt ergab Gesamtkosten von 1575 € für die ambulante und 2876 € für eine stationäre Synkopendiagnostik. Unter dem Aspekt Kosteneffizienz durch Berechnung einer virtuellen Synkopenambulanz ergab sich keine Senkung der Kosten. Der Grund findet sich in der Liegedauer und den DRG bedingten Bettplatzkosten. Zusammenfassend kann gesagt werden, dass der entwickelte Behandlungspfad in der diagnostischen Qualität vielversprechend ist. Eine Synkopenambulanz mit fokussierter apparativer und personeller Synkopenabklärung ist unter Kostengesichtspunkten nicht sinnvoll. Zukünftig könnte überlegt werden, die Synkopenabklärung in den ambulanten Sektor beispielsweise einer Poliklinik zu verlegen.

The aim of the thesis was the evaluation of a diagnostic protocol (algorithm) in order to improve syncope managment. The proposed algorithm enables a structured assessment. It is based on the recent guidelines of the European Society of Cardiology for diagnostics and therapy management of syncopes. Furthermore, the existing structure of the university medical centre Aachen (Universitaetsklinikum RWTH Aachen) was constitutive and the necessary examinations were applied stratified. The process-analysis was driven by the need to understand the effectivity of the clarfication of syncope causes by diagnostic means, which was supported by middle follow-up. To get an understanding of rising expenses the time for hospital stay at the clinic and other required resources were evaluated. Finally, a virtual syncope-ambulance is proposed and evaluated with respect to the available patient-information. The main purpose of the evaluation was to determine whether prompt care and 24h short-term surveillance leads to reduced costs. Over 6 months 131 cases of syncope-episodes were registered at the emergency-department of the clinical centre. In 100 of these cases the proposed algorithm was used as the base for the diagnostics. 68% of the cases were admitted at hospital and 32% were treated ambulatory. While 96% of these cases allowed to establish the cause of syncope, the causes of the remaining 4% stayed unclear. In the follow-up phase, 8% of the cases had recurrent syncope-episodes, but none of the ambulatory patients and none of the patients deceased. The analysis of the costs has shown, that a ambulatory diagnostic leads to costs of 467 € and the stationary diagnostic leads to costs of 2389 €. By reorganisation the diagnostics under the rules of the proposed algorithmn in a special syncope-unit more patients would be treated ambulatory without an anticipated increased risk. An analysis of the costs has shown that the total costs rise to 1575 € for ambulatory and to 2876 € for the stationary syncope diagnostics. Under the calculations of the virtual syncope ambulance no cost reduction was observed. The reasons can primarily be found in the hospital-stay and the DRG related costs for a bed. The proposed algorithm is promising under diagnostical perspective. But under costs-related criteria, a focused syncope-ambulance with dedicated technical and personal equipment is not economical. Therefore it is probably fruitful to focus future research on the evaluation of structured syncope diagnostics in ambulantory settings like Polikliniks.
