Kurzfassung
Quantitative und qualitative Analyse der Derivate der Ursodeoxycholsäure im Urin gesunder Probanden 10 gesunde Probanden erhielten über 10 Tage 750 mg Ursodeoxycholsäure (UDCA) per os. Am Tag vor Beginn der UDCA-Medikation und am Tag 10 wurde jeweils eine 24h-Urinsammlung durchgeführt. Die Sammelurine wurden über Sep-Pak C18-Kartuschen filtriert einem Gruppentrennungsverfahren zugeführt, in dem folgende Gruppen von Konjugaten bzw. folgende einzelne Gallensäuren aufgetrennt wurden: Fraktion 1 mit unkonjugierten Gallensäuren, nichtamidierten Gallensäure-Glucosiden und N-Acetyl-Glucosaminiden; Fraktion 2 mit Glycin-Taurin-Konjugaten, Glycin- oder Taurin- konjugierten Gallensäure-Glucosiden und N-Acetyl-Glucosaminiden; Fraktion 3 mit amidierten und nicht amidierten Gallensäure-Glucuroniden und Fraktion 4 mit amidierten und nicht amidierten Gallensäure-Sulfaten. Innerhalb der Konjugatgruppen wurden Cholsäure (CA) und Chenodeoxycholsäure (CDCA), Deoxychosäure (DCA) und Lithocholsäure (LCA) und Ursodeoxycholsäure (UDCA) mit ihrem Epimer iso-Ursodeoxycholsäure (iso-UDCA) quantifiziert. In der Analyse vor und nach Einnahme von UDCA zeigte sich, daß die Gesamtausscheidung der Gallensäuren mit dem Urin von 4,8 µmol/24h (1,893 µg/24h) auf 15,0 µmol/24h (5,896 µg/24h) angestiegen war. Während vor Einnahme von UDCA die Glycin-Taurin-Konjugate mit 31,3 % die mengenmäßig stärkste Fraktion darstellten, wurden sie unter der Einnahme durch die Gruppe der N-Acetyl-Glucosaminide mit 33,9 % abgelöst. Die Menge der N-Acetyl-Glucosaminide nahm von 0,50 µmol/24h vor Gabe von UDCA auf 5,10 µmol/24h zu. Insgesamt machten N-Acetyl-Glucosaminide 55,1 % aller im Urin gefundenen Metabolite der UDCA aus. Iso-UDCA wurde fast selektiv als N-Acetyl-Glucosaminid ausgeschieden. Alle weiteren Gallensäuren waren unter den N-Acetyl-Glucosaminiden nicht nachzuweisen. Der prozentuale Anteil der UDCA an der Gesamtmenge der ausgeschiedenen Gallensäuren stieg von 4 % vor Einnahme auf 49 % nach Einnahme von UDCA an, der von iso-UDCA von 3 % auf 13 %. 90 % der N-Acetyl-Glucosaminide der UDCA stellten sich außerdem als Doppelkonjugat mit Glycin heraus, 9 % der N-Acetyl-Glucosaminide der iso-UDCA waren ebenfalls doppelt mit Glycin konjugiert. Zusätzlich gelang die Identifikation eines Zwischenproduktes der Epimerisierung von UDCA zu iso-UDCA, der 3-keto-UDCA.

Quantitative and qualitative Analysis of Derivatives of Ursodeoxycholic Acid in urine of healthy human subjects Ursodeoxycholic acid (750 mg daily) was administered orally to ten healthy human subjects over a period of ten days. 24 hour urine samples were collected the day before and on the last day of the study. The samples were extracted with Sep-Pak C18 cartridges and separated into groups of conjugates The following fractions were obtained: unconjugated bile acids, nonamidated bile acid glucosides and N-acetyl-glucosaminides (fraction 1), glycine- or taurine-conjugated bile acids and glycine- or taurine-conjugated bile acid glucosides and N-acetyl-glucosaminides (fraction 2), nonamidated and glycine- or taurine-conjugated bile acid glucuronides (fraction 3) and nonamidated and glycine- or taurine-conjugated bile acid sulfates (fraction 4). Within each fraction the following bile acids were obtained: cholic acid (CA), chenodeoxycholic acid (CDCA), deoxycholic acid (DCA), lithocholic acid (LCA), ursodeoxycholic acid (UDCA) and iso-ursdeoxycholic acid (iso-UDCA). Mean total excretion of urinary bile acids increased from 4,8 µmol/24hr (1,893 µg/24hr) the day before to 15,0 µmol/24hr (5,896 µg/24hr) at day ten of the daily oral administration. At baseline 31,3 % of bile acids were conjugated with glycine or taurine, whereas after oral administration of UDCA N-acetyl-glucosaminides became the most frequent conjugates with a percentage of 33,9 % of the total bile acids. Excretion of N-acetyl-glucosaminides increased from 0,50 µmol/24hr at baseline to 5,10 µmol/24hr on day ten. In total, conjugates with N-acetyl-glucosaminide constituted 55,1 % of all urinary metabolites of UDCA. Iso-UDCA was excreted almost exclusively as N-acetyl-glucosaminide, all other bile acids were not detected among the conjugates with N-acetyl-glucosaminide. Expressed as a percentage of all bile acids UDCA rose from 4 % at baseline to 49 % at day ten, iso-UDCA increased from 3 % to 13 %. Moreover 90 % of UDCA conjugated with N-acetyl-glucosaminide turned out to be double conjugates with glycine. The double conjugates also represented 9 % of iso-UDCA conjugated with N-acetyl-glucosaminide. In the end an intermediate of the epimerization of UDCA to iso-UDCA, 3-dehydro-UDCA, was identified.
