Kurzfassung
Durch die immer größere Differenz von Patienten mit terminaler Herzinsuffizienz und der für diese Patienten benötigten Anzahl von Spenderorganen wird intensiv an Möglichkeiten geforscht, entweder die Zeit bis zur Transplantation durch künstliche Blutpumpen zu überbrücken oder eine dauerhafte Alternative zur Transplantation zu schaffen. Obwohl in den letzten Jahren deutliche Fortschritte in der Entwicklung gemacht wurden, gibt es immer noch beträchtliche Komplikationsraten bei der Hämodynamik, der Wundheilung, bei Thrombembolien und Blutungen sowie in Bezug auf technische Defekte. Aus diesen Gründen ist die Entwicklung neuer Blutpumpen dringend erforderlich. Diese Versuchsreihe bezieht sich auf die Mikrodiagonalpumpe II, eine intrathorakal implantierte nicht pulsatile Pumpe des Institutes für Biomedizinische Technik, Aachen. Über einen Untersuchungszeitraum von sieben Tagen wurde sechs Kälbern mit einem Gewicht zwischen 80-90 kg durch eine linksseitige Thorakotomie die Pumpe mit einer Förderleistung von 2-3l/min in den linken Recessus phrenicocostalis implantiert. Dabei wurde die Einlasskanüle mit dem linken Herzohr und die Ausflusskanüle mit der Aorta descendens anastomosiert. Über den Versuchszeitraum wurden täglich Blutproben entnommen. Die Leistungs- und Förderdaten der Pumpe wurden kontinuierlich erfasst. Vor und nach der Implantation sowie vor Explantation der Pumpe wurden transösophageale und transthorakale Echokardiographien und hämodynamische Messungen durchgeführt. Am Ende des Versuchszeitraumes wurden die Tiere unter Narkose euthanasiert und die Pumpe explantiert. Pumpe und innere Organe wurden anschließend makros- und mikroskopisch begutachtet. Durch eine deutliche Reduktion der echokardiographischen Daten wie HZV, LVEDD, LVESD und der EF konnte eine effektive partielle Entlastung des Herzens durch die MDP gezeigt werden. Die hämodynamischen Parameter wie MAP, MPAP, PCWP und CVP blieben während des Versuchszeitraumes stabil. Der Flow der Pumpe war über den gesamten Versuchszeitraum annähernd konstant und konnte mit Werten von 3,11 l/min bei einer Umdrehungszahl von 6000 RPM ca. 40% des Fördervolumens eines normalen Kälberherzens übernehmen. Die MDP zeigte nur eine geringe Hämolyse. Dies konnte anhand der konstanten Werte des freien Plasmahämoglobins gezeigt werden. Postoperativ kam es zu einem leichten Anstieg der Hämoglobin- und Hämatokritwerte. Bis zum Versuchsende normalisierten sich die Werte jedoch wieder. Die Endorganfunktion, gemessen durch die Blutwerte Gesamtbilirubin und Kreatinin, zeigten beide nach einem postoperativen Anstieg eine Normalisierung während der Versuchsperiode oder fielen sogar unter die Ausgangswerte präoperativ ab. Ein Grund hierfür könnten Intervalle mit niedrigerem arteriellen Blutdruck bei der Anastomosierung der Ausflusskanüle durch partielle Ausklemmung der Aorta descendens sein. Die Mikrodiagonalpumpe zeigte sich in dieser Versuchsreihe als zuverlässiges und effektives Linksherzunterstützungssystem zur Herzentlastung. Nach den viel versprechenden Ergebnissen aus dieser Versuchsreihe sind nun Langzeitstudien geplant.

Due to the increasing number of patients suffering from end-stage heart failure and the decreasing number of heart transplants the search for alternative treatment strategies is intensified. Artificial blood pumps may be used to bridge patients to transplant or might even be implanted as destination therapy. Despite considerable improvements during the last years the rate of complications remains high in regard of hemodynamic and technical failures, wound healing disturbances, thromboembolic events and bleeding complications. Therefore the development of new blood pumps is strongly encouraged. This study examines the microdiagonal pump II, an intrathoracic implanted, non-pulsatile pump which was developed at the Institute for Biomedical Engineerings, Aachen. The pump was implanted in six calves weighing 80-90 kg for seven days. It was placed in the left thoracotomy via a left-sided thoracotomy. The inflow cannula was inserted into the left atrium and the outflow cannula was anastomosed with the descending aorta. Pump flow was adjusted to 2_3l/min. Blood samples were taken daily during the examination period. Pump data were recorded continuously. Transesophageal and transthoracic echocardiographies and hemodynamic measurements were performed before and after implantation as well as before explantation of the pump. At the end of the examination period the animals were sacrificed and the pump explanted. Afterwards the pump and the inner organs were examined micro- and macroscopically. Reduction of left ventricular cardiac output, left ventricular enddiastolic and endsystolic diameters and the ejection fraction proved a partial unloading of the heart by the MDP. The hemodynamic parameters such as mean arterial pressure, mean pulmonary artery pressure, pulmonary capillary wedge pressure and cenral venous pressure remained stable during the examination period. The pump flow was almost constant over the entire examination period with 3,1 l/min by 6000 revolutions per minute. The MDP showed only minor hemolytic activity as indicated by constant values for free plasma hemoglobin. A slight increase of hemoglobin and hematocrit values was detected postoperatively. These parameters normalized until the end of the experiments. The end organ function as determined by the blood parameters for total bilirubin and creatinine was not significantly impaired. After an initial increase these values normalized or even dropped below the preoperativ values. The temporary increase can be explained by short periods of low arterial blood pressure occurring during partial clamping of the descending aorta for the anastomosis with the outflow graft. The microdiagonal blood pump proved to be a reliable and effective assist device for partial unloading of the heart. Due to the promising data from these experiments long-term studies are now indicated.
