Kurzfassung
Myeloproliferative Neoplasien (MPN), einschließlich der essenziellen Thrombozythämie (ET), Polycythaemia vera (PV) und primären Myelofibrose (PMF), sind durch eine abnormale Blutbildung, konstitutionelle Symptome und Knochenmarkversagen gekennzeichnet. Die Progression zur Myelofibrose (MF) bei ET und PV oder die primäre Fibrose bei PMF geht mit einer schlechten Prognose einher und ist nur durch eine allogene Stammzelltransplantation reversibel, die jedoch nur für eine begrenzte Untergruppe von Patienten infrage kommt. Diese Dissertation unterstreicht die Notwendigkeit nachhaltiger, nicht-invasiver diagnostischer Methoden und neuartiger therapeutischer Ansätze, die auf transkriptomischen Analysen in Kombination mit fortgeschrittener biocomputationaler Analyse für die MF basieren. Bei der Untersuchung der MF-Progression demonstriert diese Dissertation das Potenzial des Profiling von zellfreier RNA aus dem peripheren Blut, um zwischen den Transkriptomen der Prä-MF und der fortgeschrittenen MF zu unterscheiden, was neue Erkenntnisse für die Patientenschichtung liefert. Dieser Ansatz spiegelt Veränderungen in der Knochenmarknische wider und verdeutlicht eine Verschiebung hin zur extramedullären Hämatopoese in der Milz. Die Studie konzentriert sich insbesondere auf die Rolle des Transkriptionsfaktors RUNX1 bei der Fibroseprogression und identifiziert Interleukin-18 (IL-18) als einen zentralen Entzündungsmediator, wodurch neue Biomarker für die Schichtung von MPN-Patienten aufgedeckt werden. Für die chronische myeloische Leukämie (CML) entwickelt diese Dissertation einen hochauflösenden Einzelzell-RNA-Sequenzierungsatlas des Knochenmarks von CML-Patienten, identifiziert 36 Zelltypen und zeigt dynamische Störungen in verschiedenen Zellpopulationen vor und nach der Behandlung mit Tyrosinkinase-Inhibitoren (TKI) auf. Die Studie stellt fest, dass die Aktivierung des NF-κB-Signalwegs und Zytokine der IL-1-Familie Marker für Entzündungen und potenzielle TKI-Resistenz sind. Darüber hinaus deckt sie eine unerwartete Dominanz der frühen erythroiden Linie bei CML auf und identifiziert die wichtigsten Treibergene entlang der Differenzierungstrajektorie, was potenzielle therapeutische Angriffsziele nahelegt. Bei der Untersuchung des myelodysplastischen Syndroms (MDS) nutzt diese Dissertation computergestützte Modellierung, um eine Störung der Ruhezustands-Signatur in hämatopoetischen Stammzellen in einem MDS-Mausmodell aufzuzeigen. Sie demonstriert die kollaborative Rolle von TP53-Mutationen und Csnk1a1-Haploinsuffizienz bei der malignen Transformation zur akuten myeloischen Leukämie (AML) und liefert damit wichtige Einblicke in die Pathogenese der Erkrankung. Insgesamt bietet diese Arbeit einen umfassenden Überblick über die Pathogenese und Progression von drei myeloischen Malignomen. Für die MF schlägt sie ein nicht-invasives longitudinales Monitoring mittels der IL-18-Expression im Blut vor. Bei der CML hebt sie die Rolle der stromalen Nische hervor und identifiziert linienspezifische therapeutische Angriffsziele. Für das MDS klärt sie die Mechanismen der malignen Transformation auf und bietet potenzielle Biomarker und Therapieansätze. Diese Ergebnisse ebnen den Weg für verbesserte Diagnose- und Behandlungsstrategien bei diesen hämatologischen Erkrankungen.Myeloproliferative neoplasms (MPNs), including essential thrombocythemia (ET),polycythemia vera (PV), and primary myelofibrosis (PMF), are characterized by abnormal blood production, constitutional symptoms, and bone marrow failure. The progression to myelofibrosis (MF) in ET and PV, or primary fibrosis in PMF, has a poor prognosis and is only reversible through allogeneic stem cell transplantation, which is viable for a limited patient subset. This thesis underscores the necessity for sustainable, non invasive diagnostic methods and novel therapeutic approaches informed by transcriptomic analysis combined with advanced bio computation alanalysis for MF. In exploring MF progression, this thesis demonstrates the potential of cell-free RNA profiling from peripheral blood to distinguish between pre-MF and advanced MF transcriptomes, offering new insights into patient stratification. This approach mirrors changes in the bone marrow niche and highlights a shift towards extra medullary hematopoies is in the spleen. The study particularly focuses on the role of the transcription factor RUNX1 in fibrosis progression and identifies Interleukin 18 (IL-18) as a key inflammatory mediator, thus uncovering novel biomarkers for MPN patient stratification. For chronic myeloid leukemia (CML), this thesis develops a high-resolution single-cell RNA sequencing atlas of CML patient bone marrow, identifying 36 cell types and revealing dynamic perturbations across various cell populations before and after tyrosine kinase inhibitor (TKI) treatment. The study finds that NF-κB pathway activation and IL-1 family cytokines are markers of inflammation and potential TKI resistance. Additionally, it uncovers an unexpected dominance of the early erythroid lineage in CML and identifies main driver genes along the differentiation trajectory, suggesting potential therapeutic targets. In investigating myelodysplastic syndrome (MDS), this thesis uses computational modeling to reveal a quiescence signature perturbation in hematopoietic stem cells in an MDS mouse model. It demonstrates the collaborative role of TP53 mutation andCsnk1a1 haploinsufficiency in the malignant transformation to acute myeloid leukemia(AML), providing important insights into disease pathogenesis. Overall, this work provides a comprehensive overview of the pathogenesis and progression of three myeloid malignancies. For MF, it proposes non invasive longitudinal monitoring via IL-18 expression in the blood. In CML, it emphasizes the stromal niche's role and identifies lineage-specific therapeutic targets. For MDS, it elucidates the mechanisms of malignant transformation, offering potential biomarkers and therapeutic targets. These findings pave the way for improved diagnosis and treatment strategies across these hematological diseases.
