Kurzfassung
GEGENSTAND: Diese Arbeit untersucht die Ausschüttung der neurotrophen Faktoren NT3 und NGF durch sympathische Neurozyten von neonatalen Ratten bei hochfrequenter elektrischer Stimulation, sowie deren neurotrophe Wirkung über den Rezeptor TrkA, in einem in vitro Modell der Pathologie des Vorhofflimmerns. HINTERGRUND: Vorhofflimmern ist die häufigste Herzrhythmusstörung des Menschen unter der allein in Deutschland etwa 300.000 Menschen leiden. In der Pathogenese kommt es im Verlauf kardial zu strukturellem, neuronalem und elektrischen Remodelling. Es gibt Hinweise darauf, dass eine Dysbalance zwischen sympathischer und parasympathischer neuronaler Aktivität ein Auftreten von Vorhofflimmern begünstigt. Es gibt bereits Forschungsergebnisse die zeigen, dass hochfrequente elektrische Stimulation im Tiermodell zu vermehrter kardialer Ausschüttung von neurotrophem Faktor NGF und vermehrter sympathischer Innervation führt. Genaue molekulare Grundlagen sind hier jedoch noch nicht bekannt. Die genauere Charakterisierung zugrunde liegender intrazellulärer Signalkaskaden kann neue Therapiewege zur Verhinderung eines pathologischen neuronalen Remodellings bei VHF erschliessen. METHODIK: Als Grundlage der Arbeit diente ein Modell aus kultivierten sympathischen Neurozyten des Ganglion Cervicale Superior von neonatalen Sprague Dawley Ratten, welche rhythmischen elektrischen Feldern mit Frequenzen zwischen 5 Hz und 99 Hz mit einer Stimulationsspannung von 2 V/cm sowie einer Impulsdauer von 1 ms ausgesetzt wurden. Die Stimulationsdauer betrug 48 h. Vergleichsmessungen wurden mit lidocainhaltigem Zellmedium zur Verhinderung der Zelldepolarisation bei elektrischer Stimulation durchgeführt. Anschließend wurde mittels mikroskopischen Nativbildern und Immunfluoreszenztechnik das morphologische Nerve Sprouting der sympathischen Neurozyten untersucht. Außerdem wurde im Zellmedium mittels ELISA Technik die Menge an durch die sympathischen Neurozyten sezernierten neurotrophen Faktoren NGF sowie NT3 in Abhängigkeit von der Zeit und der gewählten Frequenz untersucht. PCR und Western Blot Technik dienten schließlich zur RNA und zur zellulären Proteinbestimmung von NGF und NT3, sowie ihres Rezeptors TrkA nach hochfrequenter elektrischer Stimulation. ERGEBNISSE: Diese Arbeit zeigt, dass hochfrequente elektrische Stimulation sympathischer Neurozyten, depolarisationsabhängig, durch eine vermehrte Produktion und Expression von NGF und dessen autokrine bzw. parakrine Wirkung über den, durch hochfrequente elektrische Stimulation ebenfalls hochregulierten Rezeptor TrkA, zu SNS führt.SCHLUSSFOLGERUNG: Das Verständnis der molekularen Signalkaskade bei hochfrequenter elektrischer Stimulation von sympathischen Neurozyten ist durch die Ergebnisse dieser Arbeit erweitert worden und bietet bereits Raum für erste Ideen zur effektiven Verhinderung des sympathischen Nerve Sproutings, beispielsweise durch Blockade des Rezeptors TrkA oder Depolarisationsblockade durch Antiarrhythmika der Klasse I. Im nächsten Schritt ist jedoch eine weitere Annährung des Modells an die Pathophysiologie des Vorhofflimmerns durch den Einsatz arrhythmischer elektrischer Felder oder die Entwicklung eines in vivo Modells anzudenken.

SUBJECT OF RESEARCH: This study analyzed the expression and distribution of the neurotrophic factors NGF and NT3 by sympathetic neurocytes of neonatal rats during electric high-frequency pacing and also their neurotrophic effect as a ligand of the receptor TrkA in an in-vitro model of atrial fibrillation.BACKGROUND: Atrial fibrillation is the most frequent arrhythmia only in Germany about 300.000 people suffer from. In the pathogenesis of atrial fibrillation structural, neuronal and electrical cardiac remodeling occur by time. There are references which say that a dysbalance between neuronal sympathetic and parasympathetic activity begets atrial fibrillation. Studies show, that a high frequency electrical stimulation in a canine model leads to cardiac expression of the neurotroph factor NGF and to a rising sympathetic innervation in the hart. A more detailed characterization of cellular signalling cascades could help to find new therapies to stop pathologic neuronal remodeling in atrial fibrillation.METHODS: In this study we used a model of cultured sympathetic neurocytes of the superior cervical ganglion of neonatal Sprague Dawley rats, which were exposed to rhythmic electrical fields with frequencies between 5 Hz and 99 Hz, a stimulation-voltage of 2 V/cm and a pulse duration of 1 ms. The period of stimulation was 48 h. We also made measurements with the same electrical stimulation details, with lidocaine in the medium of the cell culture, to block the cell depolarization. Afterwards we analyzed the morphologic nerve sprouting by microscopic pictures and immunofluorescence. We also characterized the neurotrophic factors NGF and NT3 in the cell medium, which were produced by the sympathetic neurocytes, as function of time and frequency with the ELISA. With the PCR and Western-Blot we could measure the RNA and the cellular proteins of NGF, NT3 and their receptor TrkA after high-frequency electrical stimulation. RESULTS: The study shows that high frequency electrical stimulation of sympathetic neurocytes, conditioned by cell depolarization, leads to sympathetic nerve sprouting by a higher expression and production of NGF and by its autocrine/paracrine effect through the also high regulated receptor TrkA.CONCLUSION: The understanding of the molecular signal cascade during high-frequency electrical stimulation of sympathetic neurocytes could be upgraded by this study and creates first ideas for the effective block of sympathetic nerve sprouting, for example by blockade of receptor TrkA or the blockade of depolarization by antiarrythmic drugs class I. In the next step of research we should think about a development of the model to get closer to the pathophysiology of atrial fibrillation, for example by using arrhythmic electrical fields or the development of an in-vivo model.
