Kurzfassung
In dieser Arbeit werden Veränderungen des Zytoskeletts von Kardiomyozyten anhand von isolierten, perfundierten Rattenherzen durch die Applikation von verschiedenen Medikamenten, welche auch untereinander kombiniert werden, untersucht. Die Effekte von Medikamenten auf das Zytoskelett, vor allen in der ischämischen Phase, werden biochemisch anhand der Kreatinkinase-Freisetzung und histologisch anhand des Apoptoseverhaltens der Myocyten untersucht. Carvedilol ist ein Betablocker mit Alpha-Rezeptor blockierender Eigenschaft und zusätzlich antioxidierenden und antiapoptotischen Effekten. Als Vergleich werden der Betablocker Propranolol und das Carvedilol-Analogon BM 91.0228, eingesetzt, welches im Vergleich zur Muttersubstanz Carvedilol, keine signifikanten betablockierenden Eigenschaften besitzt. Es werden zudem zwei das Zytoskelett beeinflussende Medikamente, Taxol und Vinblastin, untersucht. Jede der fünf genannten Substanzen wird einzeln in Hinsicht auf die Kreatinkinase-Freisetzung und das Apoptoseverhalten überprüft. Dann werden Carvedilol, BM 91.0228 und Propranolol in Kombination mit jeweils Taxol und Vinblastin eingesetzt, um zusätzliche Aussagen vor allen über die Hypoxie-Phase treffen zu können. In Bezug auf die Aktivität der Kreatinkinase führten Carvedilol, Propranolol und Taxol, nicht aber BM 91.0228, zu einer reduzierten Freisetzung der Kreatinkinase, wobei Taxol diesbezüglich den stärksten Effekt aufwies. Vinblastin zeigte ebenfalls, wie BM 91.0228, keine Reduktion der Kreatinkinase. Wurde jedoch das Vinblastin mit Carvedilol kombiniert, kam es ebenfalls zu einem Abfall der Kreatinkinase in der Hypoxie-Phase. Die Kombination von Taxol und Carvedilol führte zur signifikant stärksten Reduktion der Kreatinkinase-Freisetzung. Carvedilol, BM 91.0228 und Taxol konnten, im Vergleich zur Kontrollgruppe, die Apoptoserate um bis zu 50% senken. Vinblastin und Propranolol zeigten keinen Effekt in Hinsicht auf die Apoptose. Die Ergebnisse dieser Arbeit zeigen, dass das Zytoskelett eine wichtige Rolle bei der Enzymausschüttung während der Hypoxie-Phase spielt. Die Steigerung des kardioprotektiven Effektes des Carvedilols durch Kombination mit Taxol zeigt, dass diese beiden Medikamente eine wichtige Rolle in der Therapie der Herzinsuffizienz in Zukunft spielen könnten.

In this work changes of the cytoskeleton of cardiomyocytes are examined on the basis of isolated, perfused rat's hearts by the application of the different drugs which are also combined together. The effects of different drugs on the cytoskeleton, before all in the ischaemic phase, are examined on a biochemical basis with the help of the creatine kinase release and on a histological basis with the help of the apoptotic properties of the myocytes. Carvedilol is a beta blocker with alpha-receptor blocking quality and, in addition, antioxidative and antiapoptotic properties. As a comparison are used the beta blocker Propranolol and the Carvedilol analogon BM 91.0228, which owns in comparison to the mother's substance Carvedilol, no significant beta-blocking qualities. Besides two the cytoskeleton influencing drugs, Taxol and Vinblastin are also examined. Each of the five substances mentioned above are individually evaluated in regard of the ceratine kinase release and die apoptotic properties. Then Carvedilol, BM 91.0228 and Propranolol are used in combination with Taxol and Vinblastin to gather additional results during hypoxic conditions. Concerning the activity of the creatine kinase Carvedilol, Propranolol and Taxol, but not BM 91.0228 led, to a diminished emission of the creatine kinase and Taxol showed concerning this the strongest effect. Vinblastin also pointed, like BM 91.0228, no reduction of the creatine kinase. But the combination of Vinblatin and Carvedilol showed a significant decreased release of the creatine kinase during hypoxic conditions. The combination of Taxol and Carvedilol led to the significantly strongest reduction of the creatine kinase emission. Carvedilol, BM 91.0228 and Taxol could lower the apoptotic cell rate up to 50% in comparison to the control group. Vinblastin and Propranolol showed no effect in regard to the apoptosis. The results of this work show that the cytoskeleton plays an important role with the enzyme release during the hypoxic-phase. The increase of the cardioprotective effect of the Carvedilols by combination with Taxol shows that these both drugs could play an important role in the therapy of the cardiac insufficiency in future.
