Kurzfassung
Bei dem Magenkarzinom und anderen Tumoren ausgehend von den Hohlorganen kann es zur Ausbildung einer Peritonealkarzinose kommen, die mit einer sehr schlechten Prognose und einer mittleren Überlebenszeit, nach Diagnosestellung, von etwa sechs Monaten verbunden ist. Es gibt bei der infausten Situation keine kurative Therapieoption mehr. Deshalb kommt der Prävantion der Peritonealkarzinose eine besondere Bedeutung zu. Neben der Sogfalt des Operateurs gibt es auch verschiedene Ansätze eine intraoperative Tumorzellaussaht zu verhindern. Mit chemotherapeutisch und immunologisch wirksamen Substanzen sowie der Hyperthermie wurde versucht die Adhäsion und Migration der Tumorzellen zu verhindern. Die meisten angewendeten Substanzen zeigten eine hohe Morbidität in tierexperimentellen Studien und in der klinischen Anwendung. In dieser Arbeit wurde die Auswirkung von Phospholipiden auf die Migration von Tumorzellen im In-vitro-Versuch untersucht. Die Phospholipide zeigten in der klinischen Anwendung zur postoperativen Adhäsionsprophylaxe eine gute Verträglichkeit und eine geringe Toxizität. Eine besondere Bedeutung bei intraabdominellen Adhäsions und Migrationsprozessen wurde für bestimmten Extrazellulärmatrixkomponenten nachgewiesen. Wir untersuchten anhand eines modifizierten Boydenkammer Assays die Migration einer Magentumorzelllinie. Dafür wurde die Phospholipidemulsion in unterschiedlicher Konzentration in drei verschiedenen Versuchsanordnungen untersucht. Es wurden die migrierten Tumorzellen mikroskopisch fotographiert und ausgezählt. Die Ergebnisse zeigten eine statistisch signifikante Reduktion der Tumorzellmigration für alle drei Versuchsanordnungen. Des Weiteren wurde eine tendentielle Dosisabhängigkeit gefunden. In einem analogen Versuchsaufbau mit Dickdarmtumorzellen in unserer Arbeitsgruppe wurde ebenfalls eine statistisch signifikante Migrationshemmung nachgewiesen. Wir konnten somit die migrationsgemmende Wirkung der Phospholipide im In vitro-Versuch nachweisen.Tumors of the peritoneal cavity, such as castric cancer can cause a peritoneal carcinomatosis. Patients have a median survival of six month from time of diagnosis. The peritoneal carcinomatosis leaves no curative therapeutic option. Therefore the prevention of the PC should be the main goal. There are several diffenrent initiatives to prevent tumor cells from metastate to the peritoneum. The curativeness of the surgeon has to be the major part to keep large amounts of tumor cells from dropping into the peritoneal cavity during surgery. But evan in operations performed in so called “no touch technic” studies showed that there were living tumor cells in 90% of the probes. Several substances such as chemotherapeutics, immunologics and hyperthermia has been tested to reduce the tumor cell adheasion and migration. Many of the tested substances showed severe side effects and toxicity even in animal studies. Phospholipids showed a low toxicity and very good acceptance in clinical trials before. Goal of this work was to show the effects of phospholipids to the migration of gastic cancer cells in vitro. We tested the migration of the tumor cells at a boyden chamber assay. Therefore we pretreted the tumor cells in three different test arrangements with two different PL concentrations. The different test arrangement should imitate the peritoneal cavity with its extracellular matrix. After 24 hours we stoped the trial and counted the cells that migrated through a filter membrane with micropores. For all three test arrangement we received a statitical significant reduction of tumor cell migration. Furthermore we could investigate a trend of migration inhibition for higher doses but without statistical significance. In this study we were able to proof the reduction of migration by phospholipids in vitro. Further clinical studies shoud be performed to show the savety and evectiveness of phospholipids in vivo.
