Kurzfassung
Erst durch den Einsatz von Biomaterialen gelingt es beim Verschluß von Narbenhernien die Rezidivquoten merklich zu verringern. Man nimmt dabei Komplikationen wie, Fremdkörperreaktionen, Infektionen, Adhäsionen, Serombildung und Bewegungseinschränkung der Bauchdecke durch starre Narbenplatten in Kauf. Drei verschiedene Mesh-Materialien werden im Tierversuch analysiert: ein schwergewichtig-kleinporiges monofiles Polypropylen-Netz (Marlex), ein leichtgewichtig-kleinporiges multifiles Polyestern-Netz (Mersilene) und ein leichtgewichtig-großporiges multifiles Polypropylen-Netz mit Vicryl (Vypro). Die Materialien werden als In-lay über 3, 7, 14, 21, 90 Tage in Ratten implantiert. Die Veränderung der Bauchwandbeweglichkeit bei intraabdomineller Druckbelastung von 0-70 mmHg (0-9,81kPa) wurde mittels 3D-Photogrammetrie erfaßt. Die histologischen Unterschiede wurden vor allem in Bezug auf die unterschiedliche Menge von Bindegewebsbildung und Entzündungsinfiltrat untersucht. Die Netze mit materialreduziertem Anteil führen bei erhaltener Stabilität zu keiner nachweisbaren Einschränkung der Bauchwandkrümmung. Möglicherweise läßt sich durch derartige Netzmaterialien die Rate der lokalen Wundkomplikationen vermindern.

Only the use of biomaterials for incisional hernia repair is able to reduce the recurrence rates markedly. Disadvantages are foreign-body-reaction, infections, adhesions, local wound complications like seromas and restriction of mobility by the rigid "shell". Three different types of mesh materials are compared in a rat experiment: a heavy-weight monofilament polypropylene mesh (Marlex), a light-weight multifilament polyester mesh (Mersilene) and a light-weight multifilament polypropylene mesh with Vicryl (Vypro).The materials are implanted as an inlay in rats for 3, 7, 14, 21 and 90 days. The deformation of the abdominal wall following intraabdominal pressure of 0-70 mmHg (0-9,81 kPa) is documented by 3D-Photogrammetry. The histological differences are shown especially for the amount of connective tissue and inflammatory cells. The meshes with reduced amounts of material lead to no restriction of the abdominal wall configuration yet have uncompromised stability. It might be possible to reduce the rate of local wound complications by the use of these meshes.
