Kurzfassung
In der vorliegenden Arbeit soll die Wertigkeit einer sehr hochauflösenden dreidimensionalen T2-gewichteten Turbo Spin-Echo Sequenz (3D-T2-TSE) mit einer Matrix von 230 x 230 Pixel gegenüber einer hochauflösenden Sequenz mit einer Matrix von 128 x 128 Pixel in der Erfassung anatomischer Strukturen und pathologischer Veränderungen des Innenohrs bestimmt werden. Weiterhin wurden zwei Nachverarbeitungsverfahren, die Maximum-Intensitäts-Projektion (MIP) und die dreidimensionale Volumenrekonstruktion (3D-Rekonstruktion), in beiden Auflösungsmodi in ihrer diagnostischen Qualität miteinander verglichen.Die gesunde Kontrollgruppe von 34 Patienten wurde zum Vergleich der beiden Sequenzen und der Sichtbarkeit der anatomischen Details in den verschiedenen Verfahren herangezogen. Die diagnostische Aussagekraft der einzelnen Verfahren wurde an einem Patientenkollektiv bestehend aus 27 Patienten mit verschiedenen pathologischen Veränderungen und 26 Patienten mit einem Dehiszenzsyndrom der Bogengänge untersucht.Ergebnis dieser Studie ist eine signifikant bessere Erfassung der feinen anatomischen Strukturen, besonders der intrameatalen Nerven, in der sehr hochaufgelösten axialen Schichtaufnahme. Sie ist auch in der Beurteilung der pathologischen Veränderungen am membranösen Labyrinth signifikant besser mit weniger falsch-positiven Ergebnissen als alle weiteren Verfahren. Die MIP ist in ihrer Detailauflösung und diagnostischen Qualität stark eingeschränkt und der 3D-Rekonstruktion unterlegen. Letztere hat in der hochauflösenden Sequenz signifikant bessere Beurteilungen der gröberen Innenohrstrukturen wie die Bogengänge und deren Ampullae ergeben. Die pathologischen Veränderungen wie das Dehiszenzsyndrom der Bogengänge, das Syndrom des erweiterten Ductus und Saccus endolymphaticus und Obliterationen werden in ihrer räumlichen Ausdehnung und der beteiligten anatomischen Strukturen bestens wiedergegeben. Die MIP und 3D-Rekonstruktion der sehr hochauflösenden Sequenz sind stark von geringen Bewegungsartefakten beeinflusst und in ihrer Aussagekraft vermindert.Schlussfolgerung dieser Ergebnisse ist eine optimale Darstellung des membranösen Labyrinths mit der Kombination aus einer sehr hochauflösenden axialen Schichtaufnahme und einer hochaufgelösten 3D-Rekonstruktion. Inzwischen ist ihre Realisierung in kurzer Zeit möglich, so dass der Einbindung in den klinischen Alltag nichts mehr im Wege steht.In the present dissertation a very high-resolution three-dimensional T2-weighted turbo spin-echo sequence (3D-T2-TSE) with a matrix of 230 x 230 pixels was benchmarked in comparison to a highresolution sequence with a matrix of 128 x 128 pixels to the imaging of anatomical structures and pathological findings of the inner ear. Furthermore, we compared diagnostic quality of two postprocessing procedures, the maximum-intensity projection (MIP) and the three-dimensional volume-rendered image (3D reconstruction), when applied to both resolutions mentioned above.Based on a test group of 34 healthy patients the two imaging procedures have been compared with respect to the visibility of the anatomical details. The diagnostic significance of the different procedures was examined in a group consisting of 27 patients with different pathological findings and 26 patients with a semicircular canal dehiscence.Result of this study is a significantly better determination of the fine anatomical structures, especially of the nerves within the internal auditory canal, in the axial slices of the very highresolution sequence. Furthermore, the axial slices of the very high-resolution sequence lead to less false-positive results than all other procedures when judging the pathological findings of the membranous labyrinth. Looking to the postprocessing procedures, the MIP is inferior to the 3D reconstruction due to its strongly limited detail resolution and its limited diagnostic quality. The 3D reconstruction based on the high-resolution sequence proved significantly better judgement of the coarser inner ear structures like the semicircular canals and their ampullae. The pathological findings like the semicircular canal dehiscence, the large endolymphatic duct and sac as well as obliterations are better rendered concerning their spatial expansion and the involved anatomical structures. The MIP and 3D reconstruction of the very high-resolution sequence are strongly affected by low movement artefacts which decrease their interpretation.As a result of this study, the best representation of the membranous labyrinth is achieved in combining the axial slices of the very high resolution sequence with the 3D reconstruction of the highresolution sequence. The technical improvements of these procedures make it possible to introduce these methods into the clinical everyday life.
