abstract = {Thanks to medical progress, the therapy of adults with
congenital heart disease (ACHD) has significantly improved
resulting in both higher life expectancy and a growing
patient population. Besides physical outcome parameters, the
assessment of health-related quality of life (HRQOL) as a
measure to evaluate therapeutic success attracts increasing
attention. Methods: Patients, who were scheduled for their
regular checkups in the cardiological outpatient clinics of
the supraregional ACHD center of the university hospital in
Aachen, filled in questionnaires providing answers to the
following fields of interest: demographic data, locus of
disease and health control (KKG) and health-related quality
of life (SF36). The created files were completed by data
originating from physical examination or research on medical
records. Results: 206 patients (average age: 28 ± 10 years,
$46.1\%$ female) participated in the study. Both the
psychological and physical axis of SF36 indicated a
significantly lower quality of life in patients with
congenital heart disease compared to the normal values of
the German population. However, results suggest a rather
lowgraded reduced HRQOL in the overall cohort at
consistently low effect sizes. Positive predictors were high
                      professional qualification, positive employment status,
lower NYHA classes, an unrestricted cardiac ejection
fraction of the left ventricle and frequent physical
activity. The personal belief, that disease runs are
primarily determined by destiny revealed a negative impact
on the HRQOL, while the complexity of the underlying heart
disease did not. Conclusion: Adults with congenital heart
defect reveal a slightly reduced health-related quality of
life, whereas the complexity of the heart defect itself is
not necessarily the critical factor. Knowledge of
socioeconomic and physical/functional risk factors as well
as awareness of the individual locus of control with respect
to health and medical conditions are essential for an easier
identification of patients at risk and the determination of
                      appropriate offers for medical help.},
