The aim of the present study was to analyze and evaluate the cognitive effects of gender and vitamin D levels in subjects with schizophrenia as part of an interventional study. For this purpose, 123 subjects of both sexes with a diagnosis of schizophrenia were examined. In addition to assessing vitamin D levels, cognitive tests were carried out using the test battery (BACS - Brief Assessment of Cognition in Schizophrenia) and the TMT (Trail Making Test). There was a significant correlation between the vitamin D level, gender and cognitive performance of the test subjects. It was shown that vitamin D deficiency was associated with poorer results for most of the cognitive domains tested in this interventional study. For gender, this effect was evident for the tests of verbal fluency, processing speed and problem-solving ability. Compared to male test subjects, female test subjects had more severe impairments, particularly in the test domain of verbal fluency, depending on their vitamin D level. In summary, further studies with larger numbers of cases are needed to analyze the relationship between vitamin D levels, gender and cognitive performance in patients with schizophrenia. In addition, studies are needed to investigate the influence of vitamin D supplementation on the cognition of these patients in more detail. Should the data collected then provide indications of differences in deficits in cognitive domains depending on gender, gender-specific influences are conceivable in the future with regard to education, the selection of medication, vitamin supplementation or the type of rehabilitative support.
