Kurzfassung
Die Fortschritte beim Verständnis der Physiologie des Knochenstoffwechsels und die Möglichkeiten der Gewinnung spezieller Makromoleküle haben in den letzten Jahren dazu geführt, neue Wege bei der Behandlung von Knochendefekten einzuschlagen. Basic Fibroblast Growth Factor (100 µg), ein hochpotentes Mitogen oder das Bone Morphogenetic Protein (100 µg), dessen osteoinduktive Wirkung bekannt ist und 1 cm³ Hyaluronsäure (Hyalart 10mg/ml), ein Bestandteil der extrazellulären Matrix, wurden zu einer viskösen Gelformation miteinander kombiniert. Dieses Composite wurde entwickelt, um mittels perkutaner Injektion in Knochendefekte unterschiedlichster Art eingebracht werden zu können. In einem experimentellen Frakturmodell wurde ein Defekt kritischer Größe (Durchmesser 9,4 mm, Tiefe 10 mm) in das femoro-patellare Gleitlager von 25 Schafen gesetzt. Bei 15 Tieren wurde der Defekt entweder mit Hyaluronsäure (Ha), Ha + BMP2 oder Ha + bFGF aufgefüllt, die anderen 10 Tiere wurden als Kontrollgruppe mit einer Spongiosaplastik versehen oder als Leerdefekt belassen. Radiografische Aufnahmen und Fluoreszenzfotografien (12 Wochen postoperativ) zeigten bereits makroskopisch eindeutig, dass es zu keinem Einwachsen von neuem Knochenmaterial gekommen war. In der Kontrollgruppe hingegen wurden die Ergebnisse histomorphometrisch ausgewertet, weil es gerade bei den Spongiosaplastiken zu einem Einwachsen von reichlich Knochenmaterial gekommen war. Im Mittel ist bei den Spongiosaplastiken eine Zunahme von 19,97 % neuem Knochen zu verzeichnen, wobei der Messwert die Anteile von implantiertem Knochen und neu gebildetem Knochen zusammenfasst. Bei den Leerdefekten war es ein Zuwachs von 5,11%. Die Studie zeigt, dass Composites aus Ha + BMP2 bzw. Ha + bFGF keine geeigneten Knochenersatzstoffe darstellen. Die guten Ergebnisse der Spongiosaplastiken bestätigen den aktuellen Stand der Behandlung von Knochendefekten. Die autologe Spongiosaplastik kann, obwohl sie eine Vielzahl von Problemen mit sich bringt (limitierte Vorräte, Infektionen, Schmerzen, Blutungen etc.), immer noch als der GOLDSTANDARD bei der Behandlung von Knochendefekten angesehen werden.

In the last few years the progress made in the understanding of bone metabolism and the possibility of producing macromolecules have led to new methods being used in the treatment of bone defects. Basic fibroblast growth factor (100µg), an extremely potent mitogen or bone morphogenetic protein (100µg), whose osteo-inductive effects are well-known, and 1 cm³ hyaluronic acid (Hyalart 10 mg/ml), a component of the extracellular matrix, were combined to form a viscous gel. This composite was developed so that it could be introduced into the most diverse cases of bone defects by means percutaneous injection. In an experimental fracture model a defect of a critical dimensions (diameter 9,4mm, depth 10mm) was performed into the distal femur epiphysis of 25 sheep. In 15 animals the defect was filled either with hyaluronic acid (Ha), Ha+bmp2 or with Ha+bfgf; as a control group the other animals were either treated with autologous bone or the defect was left empty. X-ray and fluorescent photos (12 weeks after the operation) already showed macroscopically clearly that no new bone material had formed. In the control group, however, the results were analysed histo-morphometrically because precisely in the spongiosa plastic group a considerable quantity of bone material had formed. The mean value in the spongiosa plastic cases lay by an increase of 19,97% of new bone material, whereby this reading combines the quantity of implanted bone and newly-formed bone in one figure. In the cases of the empty defects there was an increase of 5,11%. The study shows that composites of Ha+bmp2 or Ha+bfgf are not useful as bone replacment materials. The positive results of the spongiosa plastics confirm the present situation in the treatment of bone defects. Although the autologous spongiosa plastic carries with it a large number of problems (limited supplies, infections, pain, bleeding etc), it can still be looked upon as the GOLDSTANDARD in the treatment of bone defects.
