Chronic-phase myeloid leukemia (CML) is driven by the BCR-ABL1 fusion kinase, yet disease progression is accompanied by extensive remodeling of the bone marrow microenvironment including vascular alterations and fibrosis. Particularly in the presence of fibrosis, poorer responses to standard therapies and a less favorable prognosis have been described. The mechanisms linking leukemic hematopoiesis to these niche alterations remain poorly understood. This thesis describes that the suppression of the transcription factor interferon regulatory factor 8 (IRF8) in hematopoietic cells is a CML-associated event that links aberrant myelopoiesis to inflammatory niche remodeling. IRF8 expression was selectively reduced in human and murine CML and restored following interferon-α therapy. Using germline Irf8-deficient (Irf8d/d) mice, we established a model that recapitulates key features of chronic-phase (CP)-CML, including myeloid expansion, extramedullary hematopoiesis, and early bone marrow fibrosis. Single-cell transcriptomic analyses revealed an inflammatory circuit in which Irf8-deficient neutrophils interact with perivascular stromal populations, including CXCL12-abundant reticular cells, pericytes, and endothelial cells. These interactions were mediated by TGF-β–integrin and S100A8–TLR4 signaling and were associated with hypoxia–related transcriptional programs and extracellular matrix remodeling. Hematopoietic-restricted Irf8 deficiency was sufficient to induce stromal activation and collagen deposition, whereas an IRF8-deficient niche enhanced BCR–ABL1–driven leukemia and increased inflammatory alarmin production. Mechanistically, IRF8 directly binds the S100A8 locus and represses the S100A8/A9 alarmin axis, establishing it as a central regulatory function. Together, findings presented in this thesis identify IRF8 as a regulator of hematopoietic-stromal homeostasis and reveal how loss of IRF8 promotes inflammatory reprogramming of the bone marrow niche during CML.
