Kurzfassung
Die Restenose nach Angioplastie stellt ein großes klinisches und volkswirtschaftliches Problem dar. Verschiedene Arbeitsgruppen konnten zeigen, dass durch den Einsatz von Radioaktivität positiv Einfluß auf die Restenosierung nach Angioplastie genommen werden kann. Der Schwerpunkt der Arbeit basiert auf der Untersuchung der Auswirkungen von 188Rhenium-Stents auf die Neointimadicke sowie den Stenosegrad und die histologischen Parameter (Neo)Endothelialisierung, Ödembildung, Apoptose, Entzündung, Thrombosierung und den Anteil glatter Muskelzellen in der Neointima. Hierzu wurden 188Rhenium Stents in Iliakalgefäße von 15 Schafen implantiert und die Ergebnisse mit denen von Kontrollstents verglichen. Zudem wurden die Auswirkungen einer zusätzlichen Gefäßschädigung untersucht. Der Einsatz radioaktiver 188Rhenium Stents resultierte in einer Verminderung der glatten Muskelzellen in der Neointima, was jedoch keinen positiven Effekt auf die Neointimadicke und den Stenosegrad zeigte. Es zeigte sich jedoch beim Vergleich der radioaktiven Stents mit und ohne Gefäßschädigung, dass bei einer zusätzlichen Gefäßläsion sowohl eine Reduktion der Neointimadicke als auch des Stenosegrades erreicht werden kann, was den Benefit des radioaktiven Labelings für diese Indikation belegt. Die (Neo)Endothelialisierung war bei den radioaktiven Stents ebenso wie bei den Kontrollstents nur schwach ausgeprägt, ohne dass jedoch gehäuft thrombotische Auflagerungen beobachtet wurden. Die Anzahl apoptotischer Zellen in der Neointima war bei den radioaktiven Stents im Vergleich zu den Kontrollstents verringert. In den Gruppen mit Gefäßschädigung zeigten sich weniger apoptotische Zellen, unabhängig davon ob es sich um Tiere mit radioaktiven Stents oder um die Kontrollgruppe handelte, als bei den Tieren ohne Gefäßschädigung. Die Ödembildung sowie die Entzündungsreaktion waren bei beiden Versuchsgruppen nur gering ausgeprägt. Durch den Einsatz von 188Rhenium-Stents kam es zu einer Verminderung des Anteils glatter Muskelzellen, wobei jedoch keine signifikante Reduzierung der Neointimadicke oder des Stenosegrades erreicht werden konnte.

Restenosis after angioplasty is a big clinical and economical problem. It has been demonstrated in different clinical studies that endovascular irradiation can reduce restenosis after stenting or angioplasty. The aim of the study was to examine the effects of radioactive self-expanding 188-rhenium stents on neointimal formation as well as on the percent area stenosis and the histological parameter endothelialization, oedema, apoptosis, inflammation, thrombosis and the grade of smooth muscle cells in the neointima. We implantated thirty stents, 15 of which were radioactive (mean activity 21,02 MBq) and 15 of which were non-radioactive (control stent), into the external iliac arteries of 15 sheep. Furthermore we investigated the influence of an additional traumatization of the vessel wall. The study showed a reduction of the smooth muscle cells in the neointima by the 188 rhenium stents. However there was no positive effect on the neointima formation and the percent area stenosis. In further studies it must be examined over a longer period of time if the inhibition of smooth muscle cells results in a reduction of neointima formation and percent area stenosis. In the group with the radioactive stent and an additional traumatization we could demonstrate a reduction of neointimal proliferation and percent area stenosis compared to the animals with radioactive stents and no traumatization. Our results suggest a benefit of irradiation in tissue with an additional traumatization. There was no difference in the endothelialization between the radioactive and the control stents. In both groups were only few endothelial cells, but no evidence of thrombosis. Compared to the control stents the number of apoptotic cells were reduced in the neointima of the radioactive stents. An additional traumatization resulted in less apoptotic cells in the radioactive as well as in the control group. With regard to oedema and inflammation there was no difference between the animals with the radioactive stents and the control stents. Implantation von 188-Rhenium stents resulted in a decreased density of smooth muscle cells in the neointima, but there is no reduction of the neointima formation and the percent area stenosis.
