Kurzfassung
Ziel der Studie war es, einen in situ aushärtenden Tricalciumphosphat-Zement in Kombination mit rhBMP-2 im Vergleich zu dem Goldstandard der autologen Spongiosaplastik zu beurteilen. Ein Trepanationsdefekt von 1.5mm in der Femurdiaphyse von Sprague-Dawley-Ratten wurde gefüllt mit TCP-Zement kombiniert mit 0.25, 2.5, oder 25 µg rhBMP-2. Die Ratten wurden nach 1 bzw. nach 3 Wochen getötet und der Femur mittels Radiographie, Histologie, Histomorphometrie und Belastungstest untersucht. Der TCP-Zement führte zu einem guten Knochenimplantatkontakt, war osteokonduktiv, biokompatibel sowie resorbierbar und erfüllte damit wesentliche Forderungen an ein Trägermaterial für rhBMP-2. Wie die intravitale Fluoreszensmarkierung zeigte begann insgesamt die neue Knochenformation im Defekt früher unter dem Einfluß des rhBMP-2. Histomorphometrische Untersuchungen zeigten nach Hinzufügen des rhBMP-2 eine verstärkte Kallusformation in der Umgebung des Defektes mit daraus resultierender erhöhter Stabilität im Dreipunktbelastungstest. Die histomorphometrischen Analysen zeigten keine rhBMP-2-dosisabhängige neue Knochenformation in den Defekten. Die Kallusformation und die Belastbarkeit der TCP + 25 µg rhBMP-2 Gruppe war der Kontrollgruppe autologe Spongiosaplastik überlegen. TCP/rhBMP-2 composites könnten sich daher als effektiver Ersatz für die autologe Spongiosaplastik erweisen.

The aim of the study was to determine bone-regenerative effects of an in situ setting tricalcium phosphate (TCP) cement combined with rhBMP-2 and to compare it with autologous bone graft. A trepanation defect of 1.5 mm in the femur diaphysis of Sprague-Dawley rats was filled with TCP cement combined with 0.25, 2.5, or 25 µg of rhBMP-2. The rats were euthanized after 1 and 3 weeks and the femur was examined by radiography, histology, histomorphometry and bending tests. The TCP-cement showed a close bone-cement contact, it was osteoconductive, biocompatible and resorbable und therewith met important requirements of a carrier for rhBMP-2. Histomorphometric analysis showed no rhBMP-2 dose-dependent increase of new bone formation in the defect. But as shown with intravital fluorochrome staining, new bone formation in the defect started earlier using rhBMP-2. Histomorphometric analysis showed a dose-dependent increase in callus formation when rhBMP-2 was added. This lead to a better stabilitiy in a three-point bending test. Callus formation and bending strength of the TCP + 25 µg rhBMP-2 group was superior to autologous bone graft. TCP/rhBMP-2 composites may prove to be an effective substitute for autologous bone grafts.
