Kurzfassung
Die Screening-Mammographie, das Standard-Verfahren für die Brustkrebs-Früherkennung, weist bekannte Limitationen auf, die eine unzureichende klinische Performance bedingen. Dies unterstreicht den Bedarf an neuen, komplementären Screening-Strategien für die Brustkrebs-Früherkennung. Hier hat die Identifizierung neuer Proteinbiomarker, welche mittels minimal-invasiver Verfahren im Blutserum detektierbar sind, ein großes Potential für die Früherkennung maligner Erkrankungen und war ein erstes Ziel der vorliegenden Arbeit. Aus einem 15 Kandidaten umfassenden Pool potentieller Serum-Proteinbiomarker konnten, basierend auf einem zweistufigen Validierungs-Modell, die Proteine AGR3 und AGR2 erstmals als neue potentielle Biomarker für die Blutserum-basierte Früherkennung des humanen Mammakarzinoms identifiziert und validiert werden. Die Kombination der ELISA-basierten Serumkonzentrationen beider putativer Marker zu einem Biomarker-Duplett, erlaubte schließlich die Diskriminierung von Brustkrebs-Patientinnen (n=32) und gesunden Spenderinnen (n=19) mit einer klinischen Sensitivität von 64,5% bei einer klinischen Spezifität von 89,5% (AUC: 0,827; KI: 0,693 bis 0,962; P < 0,001).Die onkologische Forschung verfolgt zudem das Ziel der Karzinogenese zugrunde liegende zentrale Moleküle und Signalwege zu charakterisieren, welche potentiell therapeutische Zielstrukturen darstellen bzw. deren mögliches, prognostisches Potential den Kliniker bei der Wahl individueller Therapien mit größtmöglichem Patienten-Benefit unterstützen kann. Die zu Beginn dieser Arbeit im humanen Mammakarzinom noch kaum bzw. nicht untersuchten Gene AGR3, CELSR1, CTHRC1 und NMU sollten hinsichtlich ihrer potentiellen Nutzbarkeit als prognostische Biomarker sowie ihrer möglichen kausalen Involvierung in die Karzinogenese des Mammakarzinoms analysiert werden. Es konnte erstmalig gezeigt werden, dass eine verstärkte Expression des zur Proteindisulfidisomerase-Familie zählenden Proteins AGR3 im Mammakarzinom einen möglichen, unabhängigen prognostischen Faktor für einen ungünstigen Krankheitsverlauf von G1/G2-Brustkrebs-Fällen darstellt (multivariate Hazard-Rate: 2,186; 95% KI: 1,008 – 4,740; P < 0,05). Im Einklang mit den prognostischen Daten konnte darüber hinaus ein fördernder Effekt von AGR3 auf das Zellwachstum gut differenzierter MCF7-Brustkrebszellen in einem stabilen loss-of-function-Modell herausgestellt werden (P < 0,01), was die Hypothese eines onkogenen Einflusses von AGR3 auf gut/mäßig differenzierte Brustkrebszellen unterstützt. Für den Rezeptor CELSR1 ergab eine erstmalige Subtyp-spezifische Expressionsanalyse im Mammakarzinom eine signifikante Assoziation der mRNA-Expression mit Hormonrezeptor-positiven Fällen (P < 0,001) und niedrigerem Tumorgrad (P < 0,01). Eine signifikante (P < 0,001) Korrelation einer hohen CELSR1-mRNA-Expression im Mammakarzinom mit einem verlängerten Rezidiv-freien Überleben sowie eine Stimulation des Zellwachstums (P < 0,001) infolge eines transienten CELSR1-Expressionsverlustes in gut differenzierten, luminalen MCF7-Brustkrebszellen, implizieren mögliche tumorsuppressive Effekte des Proteins im Brustkrebs. Das Glykoprotein CTHRC1 wurde bereits in einigen soliden Tumoren einschließlich des Mammakarzinoms als aberrant exprimiert und zudem als möglicher progressionsfördernder Faktor beschrieben. Darauf aufbauend konnte in der vorliegenden Arbeit erstmalig eine CTHRC1-vermittelte Stimulation der Zellmigration und damit möglicherweise eine Involvierung in Metastasierungsprozesse dieser Entität anhand eines BT549 loss-of-function-Brustkrebs-Modells demonstriert werden. Das Neuropeptid NMU wurde in allen klinisch-relevanten Brustkrebs-Subtypen signifikant (P < 0,001) gegenüber der Normalgewebe-Expression als aufreguliert identifiziert, mit der stärksten Expression in nicht-luminalen Tumoren. Interessanterweise konnte eine verstärkte NMU-Expression in größeren Brusttumoren (> pT1) sowie eine auf den luminalen Subtypen beschränkte signifikante Assoziation einer erhöhten NMU-Expression mit einem verkürzten Rezidiv- und Fernmetastasen-freien Überleben herausgestellt werden (P < 0,001 und P < 0,05). Im Einklang hiermit führte die stabile NMU-Überexpression in luminalen SKBR3-, jedoch nicht in basalen Hs578T-Zellen, zur Ausprägung eines malignen Phänotyps, charakterisiert durch eine signifikant gesteigerte Zellmotilität bei signifikant verringerter Proliferation. Hinsichtlich eines zugrunde liegenden Mechanismus, konnten auf Basis einer genomweiten Microarray-Expressionsanalyse neue NMU-assoziierte Signalwege und involvierte Gene in luminalen SKBR3-Brustkrebszellen entschlüsselt werden. So zeigte sich u.a. eine mit der NMU-Expression einhergehende Modulation des WNT-Signalweges, resultierend in einer gesteigerten Aktivierung der kleinen GTPase RAC1 und einer Expressions-Herabregulation des Transkriptionsfaktors MYC. Diese Veränderungen könnten zur Entstehung motiler, gering proliferierender Krebszellen beitragen, die ein erhöhtes Potential zur Ausbildung von Metastasen aufweisen.Zusammenfassend ist es gelungen mit AGR3 und AGR2 zwei neue putative Proteinbiomarker für die Blutserum-basierte Brustkrebs-Früherkennung zu identifizieren. Darüber hinaus konnte insbesondere mit NMU ein möglicher neuer progressions-assoziierter Faktor im humanen Mammakarzinom charakterisiert werden, welcher möglicherweise das Potential für eine klinische Nutzung als prognostischer Parameter bzw. als therapeutische Zielstruktur aufweist.

Mammography has become the standard of care for breast cancer screening although several limitations are known resulting in reduced clinical sensitivity and specificity. Owing to these limitations minimally-invasive novel screening tests are desirable to complement screening mammography. As measurement of protein biomarkers present in bodily fluids (e.g. serum) constitutes a promising tool for early detection of breast cancer, the identification of novel candidates was an aim of this study.Based on a two-step validation procedure using human tissue and serum cohorts of human breast cancer (n=40) and healthy serum samples (n=40) we identified AGR3 and AGR2 from an initial pool of 15 candidates as novel potential protein biomarkers for blood-based early breast cancer detection. The combined AGR3/AGR2 biomarker panel achieved a clinical sensitivity of 64.5% and a specificity of 89.5% as shown by receiver operating characteristic (ROC) curve statistics (AUC: 0.827, CI: 0.693 – 0.962, P < 0.001).Besides the aim of finding novel biomarkers for early diagnosis of cancer, molecular oncology research focuses on deciphering the molecular events underlying carcinogenesis. The identification of key molecules and signalling pathways of malignant cancer cells offers the opportunity to develop novel targeted cancer therapies. Furthermore, novel prognostic biomarkers are desirable, able to support the clinician’s choice of a personalised therapy with a maximal benefit for the patient. Therefore the second focus of the present study was on the analysis of the putative usability of the novel cancer-associated genes AGR3, CELSR1, CTHRC1 and NMU as prognostic factors as well as their possible causal involvement in the oncogenesis of human breast cancer. High expression of the protein disulfide isomerase family-related protein AGR3 was significantly associated with an unfavourable outcome (multivariate hazard ratio: 2.186, 95% CI: 1.008 – 4.740, P < 0.05) in low (G1) and intermediate (G2) grade breast carcinomas indicating for the first time an independent prognostic impact of AGR3 in breast cancer. In line with the prognostic data, it could be demonstrated for the first time that stable knockdown of endogenous AGR3 protein significantly (P < 0.01) reduced cell growth of well differentiated, luminal MCF7 breast cancer cells suggesting a tumour-promoting function of AGR3, at least in well to moderately differentiated breast carcinomas. Subtype-specific expression analysis for the adhesion G protein-coupled receptor CELSR1 in human breast cancer revealed a significant correlation between high CELSR1 mRNA expression and both a positive hormone receptor status (P < 0.001) and lower tumour grade (G1/G2) (P < 0.01). Furthermore, univariate survival analysis showed that high CELSR1 mRNA expression significantly (P < 0.001) correlated with prolonged recurrence-free survival in breast carcinomas. In line, a transient knockdown of CELSR1 in luminal MCF7 breast cancer cells led to significantly increased cell growth (P < 0.001) suggesting altogether tumour-suppressive properties of CELSR1 in breast cancer.The secreted glycoprotein CTHRC1 has recently been described as aberrantly expressed in several human malignancies including breast cancer as well as a factor potentially stimulating cancer cell migration, invasion and consequently metastasis. Here CTHRC1-mediated promotion of cell migration in motile BT549 breast cancer cells was reported for the first time indicating a putative involvement of CTHRC1 in breast cancer progression.Conducting a subtype-specific mRNA expression analysis, the neuropeptide NMU was found being significantly (P < 0.001) up-regulated in all clinically relevant breast cancer subtypes compared to normal breast tissue controls, with the highest mRNA expression observed in HER2-enriched and basal-like carcinomas. In accordance with the recently described potential involvement of NMU in the progression of several malignancies, a pronounced NMU expression was noted in advanced breast carcinomas of larger size (> pT1). In further support of this notion, high NMU mRNA expression was significantly associated with reduced recurrence-free (P < 0.001) and distant metastasis-free survival (P < 0.05) of patients with luminal but not non-luminal carcinomas. In compliance with these findings, ectopic NMU over-expression facilitated a malignant phenotype in luminal-like SKBR3 but not basal-like Hs578T breast cancer cells. Stable NMU over-expressing SKBR3 single-cell clones were marked by a significantly enhanced cell motility and a reduced proliferative activity (for both P < 0.001) compared to empty vector control cells with low NMU expression. With regard to underlying mechanisms, NMU-associated pathways and putative involved key players were identified based on whole genome expression analysis employing the luminal SKBR3 breast cancer model. For example a NMU-induced modulation of WNT receptor signalling amongst other cancer-relevant pathways was observed, resulting in an enhanced activation of the small GTPase RAC1 as well as a down-regulation of MYC expression in SKBR3 NMU clones compared to controls. In turn, NMU-based effects on luminal breast cancer cells are potentially mediated by the receptor NMUR2. These findings may indicate a contribution of NMU to the development of a motile tumour cell phenotype with an increased potential to metastasise.In summary, this study shows for the first time the potential usability of AGR3 and AGR2 as biomarkers for non-invasive early detection of human breast cancer from blood, while NMU was characterised as a new progression-associated factor in breast cancer.
